A treatment designed for people who have already tried the standard path
Most antidepressants act on serotonin, norepinephrine, or dopamine, and take four to six weeks to show their full effect. Esketamine works through the glutamate system and can begin to lift symptoms considerably faster.
That difference matters most for patients with treatment-resistant Major Depressive Disorder — those who have cycled through multiple medications and started to believe improvement isn't possible for them.
Every dose is given in our office, in a calm private room, with our staff present for the entire monitoring period. You are never asked to manage this treatment on your own at home.

- What is Spravato®?
- Spravato® (esketamine) is an FDA-approved nasal spray derived from ketamine. It works on the brain's glutamate system rather than the serotonin pathways targeted by most antidepressants, which is why it can help people who have not responded to conventional medications.
- Who is a candidate?
- Adults with major depressive disorder who have tried at least two oral antidepressants at an adequate dose and duration without sufficient benefit. A full psychiatric and medical evaluation determines candidacy.
- How quickly does it work?
- Some patients notice improvement within the first week or two — faster than typical antidepressants. Others need the full induction course. We measure response with standardized rating scales rather than impressions alone.
- Is it covered by insurance?
- Many commercial plans, TRICARE, and Medicare cover Spravato® when criteria are met. Our office manages prior authorization and will review expected costs with you before treatment begins.
- What are the side effects?
- The most common effects are dissociation, dizziness, nausea, sedation, and a temporary rise in blood pressure — usually during the monitoring window and resolving the same day. Spravato® is only available through a restricted REMS program, which is why it must be given in a certified office.
- Can I keep my other medications?
- Yes. Spravato® is used alongside an oral antidepressant, and your existing treatment plan continues under our management.
